Welcome to the Consolidated Football Federation. All Pop Warner Associations in the Consolidated Football Federation League are responsible for theri own registration process. Each is allowed to charge a registration fee. This fee varies at each association. Additionally, each association schedules its own participant registration dates. To register your children for one of our Pop Warner Associations and for them to be permitted to practice, you will need the following information:
PATICIPANT CONTRACT AND PARENTAL CONSENT - This Pop Warner document is the basic registration form and contract signed by parents agreeing to the rule of Pop Warne.
PHYSICAL FITNESS & MEDICAL HISTORY FORM - This Pop Warner form is a signed statement from an examining physician licensed in North Carolina that a candidate is physically fit and there are no observable conditions which would contraindicate his or her participating in football or cheerleading. PLEASE NOTE, THE PHYSICAL EXAMINATION MUST BE DATED WITHIN THE CURRENT CALENDAR YEAR - (AFTER JAN 1 of the current year).
PROOF OF AGE - A CERTIFIED copy of the birth certificate on file bearing the seal of the issuing office of the state of birth is preferred. Passports may be used as well as Military ID cards. It is extremely important to note that photocopies of originals or any other alleged proof of birthdate are accepted only upon the willingness of a team to have its schedule forfeited should fraudulent application later be determined.
SCHOLASTIC FITNESS - The MOST RECENT COMPLETE Report Card (Containing all four quarters of grades and the final grade) and a completed Pop Warner Scholastic Form.
